Parental Consent User Guide

 Audience: Staff who verify donor consent at drives (team leads, collections specialists, screeners). For background, see the Overview page. For admin tasks (QR code, disclosures, logo, access), see the Admin Guide

 1. Logging in and opening the module

  1. Go to Lab Symplified and log in with your username/password or SSO.
  2. On the home page, select the Parental Consent menu.
  3. You land on the Donor Dashboard, your main working area.

Don't see the menu? The Parental Consent menu only appears if your account has the Access Parental Consent role. Contact your system administrator to have it added.

2. How donors sign up (background)

You don't sign donors up yourself, but understanding the flow helps you assist donors at the table:

  1. The donor scans the static QR code (on a flyer, poster, or a public web page) and the sign-up portal opens.
  2. Step 1: donor first/last name, donation site (free text), drive date, and date of birth. Donors below the minimum age on the drive date cannot continue.
  3. Step 2: the parent/guardian's information, including the guardian's email. The donor submits and sees a confirmation.
  4. The system emails the guardian a consent request, and the record appears on your Donor Dashboard as Pending.
  5. The guardian clicks the button in the email, reviews the consent form, types their name to consent, and the record becomes Signed.

3. Searching the Donor Dashboard

  • Search by name — look up a donor by first or last name.
  • Filter by site — type a donation site to pull up every sign-up for that site, then narrow by a specific drive date.

Each record shows the donor, donation site, drive date, and consent status.

4. Verifying a donor at the drive (Donor Lookup)

This is the key check-in task. When a donor arrives:

  1. Open Donor Lookup.
  2. Search by last name (first name or date of birth also work).
  3. Review the match: donation site, drive date, guardian info, and consent status.
  4. If Signed → the donor is cleared. Download the consent PDF if you need a copy.
  5. If Pending → follow Section 5 to confirm the email and resend.

5. Correcting an email & resending consent

A Pending record means the consent has not been provided. This could be due to a wrong/mistyped email or a message stuck in spam. If this happens, take below steps.

  1. Find the record and open the Update menu.
  2. Confirm the guardian email with the donor. If it's wrong, correct it — the system then prompts you to resend to the new address.
  3. If the email was already correct, resend it; a new email goes out instantly. Ask the donor to have their parent check spam/junk.

Last-resort workaround: If consent still can't be obtained (e.g., a glitch), have the donor scan the QR code and sign up again on the spot, then have the parent sign the new consent. If this creates a duplicate, archive one of them (Section 7).

6. Marking a donation as collected

  1. After a donor with Signed consent donates, open their record and select Mark as Collected.
  2. The record moves from the active dashboard to history with a status of Collected.

Automatic cleanup: If you forget, a Signed record automatically moves to history 72 hours after the drive date.

7. Archiving a canceled drive

  1. If a drive is canceled (e.g., weather), archive all sign-ups for that day — signed or not — in one action.
  2. An archived consent cannot be reused. If the drive is rescheduled to a different date, donors must sign up again and obtain a new consent.
  3. Archiving is also how you remove a duplicate record.

8. Downloading a consent PDF

From a record, download the consent as a PDF. It mirrors the paper form and shows the signature, the signing date/time, and supporting details at the bottom — print it if you need a hard copy.

9. Reports & downtime backup list

You can filter consent records and export them for reporting or as a downtime backup:

  • Filter by donation site and drive date.
  • Export to PDF — a printable list of donors with drive date, site, and consent status (Signed / Pending). Print this before a drive so you can verify consent even if you lose connectivity on-site.
  • Export to Excel — the same data in a spreadsheet for additional filtering.
